Once you have been in touch with us and are registered over the phone, you will receive a link several days prior to your initial appointment, to fill out several initial forms necessary prior to your visit. Hard copies will be available for your convenience if you would prefer, on the day of your arrival. It is preferable that these be completed prior to your visit so that we can be sure to start your appointment on time. Please plan to arrive approximately 15 minutes prior to your first visit to our practice to ensure we have everything we need before starting any treatment. If your first visit is for a dental emergency, only the tooth in question will be addressed. Depending on the diagnosis, an additional appointment with adequate time may be necessary.
